MadWorld - after being released exclusively for the a "family friendly" Wii console, critics were all too happy to deride this game for being over the top gory for no real purpose. -

Custer's Last Stand - released on the Atari, this game came under fire from women's groups for simulating rape and by Native Americans for shamelessly insulting their culture but nonetheless became one of the best selling adult games every released. -

Call of Duty: Modern Warfare 2 - in an optional mission, a terrorist group mows down a crowd at an airport sparking World War III. This led to the mission being removed from certain countries and the game being banned in others. -

Postal 2 - reveling in its sophomoric humor, this game allowed players to decapitate bystanders and urinate on them and featured animal cruelty, leading to it being banned in several countries. -

RapeLay - a twisted game in which the player stalks a family and rapes them, this game was restricted from being sold outside of Japan. -

Thrill Kill - this game was about 95 completed before EA decided to pull the plug and cancel its release because of its over the top violence and SBDM overtones. A disgruntled programmer leaked the source code onto the internet which allowed people to play it using emulators.
